Client Record Requests

Legal guardians of clients under age 18, as well asconservators for conserved clients, may access all available medical records. Clients age 18 and older who are not conserved may also access copies of their own medical records.

Clients can access medical records by logging into CentralReach (Login Instructions FAQ). Additional records may also be available through the Client Connect Portal (Login Instructions FAQ).

If a client cannot access records on their own, they may submit a request using the following form: Client Medical Record Request Form

  • ABA assessments and progress report
  • Session notes and data summaries
  • Authorizations and service logs
  • Behavior plans

Please note: Internal documents, internal emails, corrective actions related to incidents, and staff personnel information are not part of the client medical record and will not be released.
